The ice caps have fully melted now and the polar bear has nowhere to go! So it evolved to live in the water. Its hind legs were replaced with one big fin to help it swim since it will spend most of its time in water, its front legs stayed the same but got webbing to help its swim. The front stayed the same so it could catch prey. It also lost its tail to have better arrow dynamics and its fur got thinner to glide in the water. Its nose also was replaced with gills to breathe underwater. It also lost its ears so they won't get clogged.
